Piriformis syndrome

Piriformis syndrome (sciatica) indicates that there is irritation of a large nerve that runs through the back through the entire leg, the sciatic nerve. This nerve runs down the leg from the spine. However, before he gets to the leg he has to pass some glutes. For example, it runs through the m.piriformis, although there are many variations on this (it can be below, above or through). Phase 1 of revovery

During this phase the goal is to reduce the muscle tension by stretching the gluteal muscles and lower back.

1. Rek Bilspieren en Lage rug

12x • 3 sets

Ga op je rug liggen. Buig het been van de te rekken kant. Pak de knie vast van het been. Trek vervolgens de knie naar de kant van het nog gestrekte been en richting de borst. Houd de positie vast.

2. Rek Bilpsieren

12x • 3 sets

Leg het te rekken been gebogen op een bankje met de voet van het bankje af. Leg je onderarmen voor het been en buig naar voren toe met het bovenlichaam. Houd de positie eventjes vast.

3. Rek Bilspieren

12x • 3 sets

Ga zitten op een bankje of stoel. Breng het te rekken been over het andere been met de knie naar buiten wijzend. Druk met je arm je knie naar beneden en leun met het bovenlichaam iets naar voren toe. Houd deze houding vast.

4. Één been langer maken

15x • 3 sets

Ga op de rug liggen, zorg dat de knieën gestrekt blijven, duw vervolgens één been weg (maak één been langer dan de andere), wissel deze beweging af per been.

5. Knieën links rechts laten vallen

15x • 3 sets

Ga op de rug liggen, buig de knieën en zet de voeten op de grond, laat de benen langzaam naar rechts vallen en vervolgens naar links vallen.

6. Één been naar de borst

15x • 3 sets

Ga op de rug liggen, pak met de handen één been vast, trek deze vervolgens richting de borst, wissel de benen continue af.

7. Beide benen naar de borst

15x • 3 sets

Ga op de rug liggen, pak met de handen beide been vast, trek deze vervolgens richting de borst.

8. Kwispelen

15x • 3 sets

Neem plaats op handen en knieën, zet de benen iets uit elkaar, beweeg de billen naar links en naar rechts, wissel dit af.

9. Bekken kantelen

12x • 3 sets

Ga op een stoel zitten, zak vervolgens helemaal onderuit en strek je vervolgens weer helemaal op.

Phase 2 of recovery

During this phase, the goal is to strengthen the tendon and muscles through exercises.

1. Bruggetje maken

15x • 3 sets

Ga op de rug liggen, buig de knieën en zet de voeten op de grond, kantel het bekken naar achteren, span de buikspieren aan, kom vervolgens met de billen omhoog.

2. Bruggetje maken één been

15x • 3 sets

Ga op de rug liggen, buig de knieën en zet de voeten op de grond, kantel het bekken naar achteren, span de buikspieren aan, kom vervolgens met de billen omhoog. Strek vervolgens 1 been uit.

3. Superman op handen en knieën

12x • 3 sets

Ga op de je handen en knieën zitten, maak de rug helemaal recht (bol), strek vervolgens één arm en gekruist het been uit, breng de arm en het been weer terug.

4. Clam shell

15x • 3 sets

Lie on your side with your shoulder and pelvic girdles aligned vertically, using your upper arm as an anchor against the surface. Bend your hips to 45 degrees and your knees to 90 degrees, keeping your feet together and slightly lifted off the surface. Move your top knee outward/upward while keeping your feet together, then close it again. Your pelvis must remain vertical.

5. Abductie in zijlig

15x • 3 sets

Ga op de niet aangedane zijde liggen, draai het bovenste been naar binnen en breng het been een stuk omhoog. Let op de heupen en het bekken mogen niet naar achteren draaien.

Phase 3 of recovery

During this phase, the goal is to build up to sports activities.

1. Squat

12x • 3 sets

Place your feet hip-width apart and perform the movement as if you were sitting down on a chair. Push your hips back and keep your knees behind your toes. Rise back up and repeat the movement.

2. Standing lunge

12x • 3 sets

Perform a forward lunge. Step forward with one leg. Lower the other leg towards the ground, though it does not need to touch it. Ensure that the knee of the leg you stepped forward with does not extend past your toes.

3. Hip thruster 1 been

12x • 3 sets

Hip Thruster 1 been, steun met je schouders op een verhoging. Leg een stang (met gewicht) aan de voorkant van de heupen, plaats de voeten dicht bij de verhoging. hef 1 been van de grond en kom vervolgens met de billen volledig omhoog, herhaal de beweging.

4. Deadlift

15x • 3 sets

Ga op heupbreedte staan met een stok of barbell in de hand. Zak, met de billen naar achteren en de stok/barbell via de benen, naar beneden. Kom vervolgens explosief omhoog waarbij de rug recht blijft.

5. Gluteus medius walk (elastiek boven de knie)

15x • 3 sets

Ga iets breder dan heupbreedte staan met een elastiek om beide benen boven de knieën, houdt tijdens de oefening spanning op het elastiek, zak een klein stukje door de knieën en loop nu zijwaarts op en neer.

The meaning of a piriformis syndrome

Piriformis syndrome (sciatica) means there is an irritation of the large nerve (sciatic nerve) that runs from your lower back down the back of the entire leg. Before reaching the leg, the nerve has to pass through the gluteal muscles. It runs through the piriformis muscle although variations of its path are possible (it can run through, under or over the piriformis muscle). Piriformis syndrome is caused when the sciatic nerve is impinged by the piriformis muscle.

The causes of a piriformis syndrome

This syndrome is caused by overuse or weakness of the lower back muscles. The gluteal muscles have to work harder to maintain lower back and pelvic stability. This leads to increased muscle tone in the gluteal muscles causing pain with sitting, putting your shoes on or reaching to the ground. 

The symptoms of a piriformis syndrome

Piriformis syndrome usually causes local pain in the gluteal muscles that can radiate down the back of the leg and even into the foot. In more severe cases tingling, pins and needles and numbness can be experienced. Sitting on the gluteal muscles is often uneasy or painful.

The treatment of a piriformis syndrome

You can start with exercises and stretches to reduce the muscle tension in the gluteal muscles. The physiotherapist can use manual techniques to further reduce the stiffness. Recovery times for piriformis syndrome range from 3 weeks to 6 months.

Did you know?

Scientists are unsure if solely the piriformis causes this injury. There are a lot of muscles in the gluteal region and this syndrome is now often referred to as ‘deep gluteal syndrome’, meaning the pain is caused by some of the deeper gluteal muscles.
